The nurse should check whether the IV line is clogged. Four hours is too fast for infection so the swelling probably is caused by a blockage of the fluid. The nurse can try to correct the position of IV line or flush NaCl to the infusion to remove the clot.
If the IV line clot and cannot be fixed, the IV line should be removed and changed to another site.
